Welcome!
Chatham-Kent is a ‘community of communities’ composed of many small towns and villages in the heart of southwestern Ontario. Located between Lake Erie and Lake St. Clair, Chatham-Kent is a welcoming and diverse rural community.
On December 7, 2019, the Ministry of Health officially announced the Chatham-Kent Ontario Health Team (CKOHT) as one of the first 24 Ontario Health Teams in the province. Today, there are 54 organizations and over 100 individuals that participate formally in the CKOHT as a representative on one or more of the: Collaborative Steering Committee, three Advisory Councils (Physician, Board Chairs of the Steering Committee Organizations, Patient and Family Advisory), seven Working Groups and several Sub-Working Groups, called Action Teams.
CKOHT’s Action Teams, Advisory Councils and Collaborative Steering Committee are all focused on accomplishing the goals and objectives as set out by CKOHT’s Strategic Plan 2021-2024, in order to achieve the best health and well-being together.

We respectfully acknowledge that the land known as Chatham-Kent today is the traditional land of the Three Fires Confederacy: The Odawa, Potawatami and Ojibwe. We recognize and are grateful for the Indigenous Peoples who have lived, worked, and cared for this land since time immemorial. We are committed to building meaningful relationships and working together to improve health and well-being for all members of our community.
