2025 Year in Review Report

In 2025, the CKOHT partnership had a successful year, making significant progress through strong collaboration across the health system. Key achievements included advancing system integration and improvement by enhancing navigation, coordination, efficiency, and responsiveness, as well as improving health outcomes and expanding access to primary care so more individuals could connect with a family physician or nurse practitioner. The partnership also prioritized advancing health equity by fostering a more inclusive, culturally responsive, and accessible care environment. Additionally, the launch of the 2025–2030 CKOHT Strategic Plan established a clear, shared vision and set priorities to guide ongoing system transformation.
