HSPN Data

The Health System Performance Network (HSPN) is a group of researchers and evaluators working out of the University of Toronto that supports the health system in Ontario, including the Ontario Health Teams (OHT). HSPN has prepared and provided each OHT with a data package that provides their local data and compares their performance to the province.

HSPN has produced a set of 28 indicators for OHTs that include common populations of interest (Mental Health, Older Adults, Palliative and End of Life). HSPN has presented these indicators by various stratifications including primary care attachment, material deprivation, CIHI Population Grouper, and BC Health Matrix.

These HSPN data packages can be accessed here:

In April 2024, the CKOHT presented the HSPN Data packages to partners. These sessions provided an overview and local context of the CKOHT’s current performance for each indicator. You can access the presentation slide decks and recordings below.

The CKOHT’s Data Symposium Sessions were recorded and can be accessed below:

Session 1: Population Health Indicators

Session 2: Mental Health and Addictions

Session 3: Older and Frail Adults

The slide deck for all sessions can be accessed here
