First Five Program

Providing preventative and health promotion services to children aged 0-5 who do not have a primary care doctor or nurse practitioner
You do not have to live in Chatham-Kent to access services for First Five. Clinics will be offered at the Chatham-Kent Community Health Centres Chatham, Wallaceburg and Walpole Island sites. Services at outreach locations throughout Chatham-Kent are also offered as needed.
Translation services are available for non-English speaking families.
Community partners, parents and/or caregivers of children can refer to the program.
If you have a child who meets eligibility criteria and are interested in the First Five program, please complete the Registration Form found on the CKCHC website and a member of the team will call you to discuss further.
NOTE: We do not accept referrals for current pregnancies/unborn infants. Please send us a referral after your child is born.
