Improving Care for Patients with COPD

The CKOHT is committed to creating integrated care pathways that can support community members with COPD, their care partners, and healthcare providers.

CKOHT partners, community members with lived experience, and healthcare providers are working together to accomplish the following goals with this project:

• Standardize referral processes to specialized COPD care when community members are discharged from the hospital
• Standardize referral to specialized COPD care and diagnostic tests in primary care
• Providing supports to community members with COPD that could benefit from additional social and community-based services to help manage their health
