Improving Palliative Care in Chatham-Kent

The CKOHT and its partners are aiming to improve palliative care delivery for individuals with any life-limiting diagnosis living in Chatham-Kent requiring home and community care support services.

What can patients expect from being in this project?

As part of this test of change, patients will be connected to a Palliative Care Clinical Coordinator (PCCC) much earlier in their disease progression. The PCCC will coordinate their home care from referral to end of life. Additionally, the PCCC will help patients and their families and/or caregivers with system navigation to ensure they are receiving supportive services and experience smoother transitions.
Who is eligible for this program?

Given this is a test of change, patient eligibility is limited to those who are rostered to the Chatham-Kent Family Health Team (CKFHT) or Chatham-Kent Community Health Centers (CK CHC). Patients will also need to have a diagnosis for a life limiting illness who could benefit from a palliative care approach to care. Lastly, patients need to reside in Chatham-Kent.
