Let’s Go Homeward Bound (LEGHO)

The Let’s Go Home with Homeward Bound (LEGHO) program, led by the Canadian Red Cross in partnership with Chatham-Kent and Sarnia-Lambton Ontario Health Teams (OHTs), supports older adults transitioning from hospital to home.

Available for eligible clients aged 55 and older, LEGHO offers up to six weeks of services, including transportation, frozen meal packages, meal preparation, shopping, light housekeeping, laundry, wellness checks and more—all at no cost to the client. This service aims to improve clients’ confidence in self-managing at home while reducing hospital readmissions and emergency visits.

Referrals are accepted from multiple sources, and follow-up calls connect clients to ongoing community support. To qualify, clients must be medically stable, willing to receive services, and face a risk of readmission due to limited family or social support.
