Community navigation is the process of connecting citizens to support structures within public services, social services, healthcare or the wider community. It involves outreach, education, and technical assistance to increase awareness of, and participation in, local programs. When people come to your organization, they may have additional or complex needs that may require services and programs from two or more agencies. Do you ever wonder which organizations would be best to call to provide the best referral for the client? Do you wish you had one place to go to collaborate and find the best path for the client? The CoP Navigation Portal provides a secure space to work across partnerships to find the right resources and support for those who need help!

Join the Navigation Community of Practice (COP)

We invite all those who navigate on behalf of community members to join the Community of Practice Navigation Portal. The CoP Navigation Portal will allow service providers to connect, share, and collaborate to best support those that they serve. The portal will also provide increased access to resources, education, and much more!

Follow the ‘Click To Join’ link below to the HealthChat Login Page. If you are a new user, select ‘Register’ and search for the ‘Chatham-Kent Navigation Community of Practice’ or type ‘CKOHT’ in the search bar. Click on the blue “Join this Group” button and complete the registration! Group Administrators will verify all requests to join. Once access is granted, you will be able to utilize all the features within our collaborative navigation space!
